Jack treats individuals with functional limitations due to a wide variety of neuromusculoskeletal conditions affecting the spine, hip, knee, ankle, foot, shoulder, and elbow. He sees a variety of functional limitations utilizing manual and exercise-based interventions into a patient-centered approach.
He earned his Doctor of Physical Therapy degree from Carroll University. He is licensed by the state of Wisconsin for physical therapy. He is participating in specialization through the University of Wisconsin Hospitals & Clinics and UnityPoint Health – Meriter Orthopedic Physical Therapy Residency. He sees patients via provider referral. He is a member of the American Physical Therapy Association (APTA) and APTA-Wisconsin.
