The following article is from the Advanced Heart Failure referring provider newsletter.

The Virtual Advanced Heart Failure Clinic provides follow-up telehealth appointments for existing and stable patients who have been identified by our team as appropriate for virtual care. Through secure, HIPAA-compliant video visits, eligible patients can receive expert follow-up care without the need to travel to our Madison clinic.

This clinic allows some patients to avoid travel while maintaining continuous management of their condition—critical factors in improving health outcomes.

How it works

Patients are identified for participation in the Virtual Heart Failure Clinic based on their clinical stability and ongoing care needs. These patients will be contacted by our team to schedule a telehealth appointment. We will help guide patients through the setup process to ensure a smooth, reliable video visit experience.

During these virtual visits, patients receive the same comprehensive care they would in the office, including medication management, symptom assessment, lifestyle counseling and coordination of necessary diagnostic testing. Our multidisciplinary team remains fully engaged in their care plan, leveraging telehealth to maintain close monitoring and timely interventions.

Benefits for referring physicians and patients

  • Improved access

Qualifying patients can attend follow-up visits without the logistical challenges of travel, reducing missed appointments and enhancing adherence to treatment plans.

  • Continuity of care

Virtual visits support ongoing management and early detection of potential complications, helping to prevent hospital readmissions.

  • Patient-centered convenience

This service offers appointment flexibility, especially important for those with mobility issues or travel limitations.

We encourage you to inform your patients about this new option for heart failure follow-up care.
