Heather Boehme, NP

Heather Boehme, NP, is a UW Health nurse practitioner. She cares for patients in skilled nursing facilities (SNFs) for long-term care. These facilities help bridge the gap between hospital stays and returning home. Patients receive therapies to support their recovery after illness, injury or surgery. With a clinical interest in geriatrics, Heather especially enjoys working with older adults. 

Once patients arrive at their skilled nursing facility, Heather meets with them within three days. Together, they review the hospitalization and identify any issues that might have occurred during the transfer process. Then, she coordinates care with the nursing team. 

Heather sees patients in SNFs during their admission and discharge home. If they have a sudden change in their condition, she helps provide timely care to keep their recovery on track. Her support is an extension of each patient’s primary care team. She communicates all visits with primary care providers to keep them informed at every step. 

Heather is board-certified as a family nurse practitioner (FNP-BC). 

I regularly check in with patients and nursing teams to make sure nothing slips through the cracks during care transfers. No one should feel lost or uncertain about what’s next.
