Haleigh Moss, NP, is a UW Health nurse practitioner who specializes in geriatrics. She cares for patients with a wide range of acute or chronic conditions, including high blood pressure, diabetes, heart failure, kidney disease and respiratory conditions.
Her patients are over the age of 65 and are in a nursing home or receiving rehabilitation services following a recent hospitalization.
She works with nursing home staff to facilitate a positive patient transition from hospital to rehabilitation care and then back home.
