Colleen Foley, NP, is a UW Health nurse practitioner who specializes in geriatric (over the age of 65) health care.

After a hospitalization, a patient will often need to receive additional care at a skilled nursing facility before they can go home. Colleen works with a patient’s primary care team throughout this stay to make sure all needs are met, any new health concerns are addressed, medications are managed and then she helps the patient transition to home when they are ready. She cares for patients with a wide range of conditions, including high blood pressure, heart failure, chronic kidney disease, respiratory conditions, urinary tract infections, wounds and skin infections. She also offers palliative care and hospice care.

Colleen was inspired to become a nurse by her grandmother and grandfather who were both nurses. She provides compassionate and personalized care using evidence-based practices.

Outside of work, Colleen enjoys spending time with her family, traveling, playing piano and singing.
