Keeping Patients Healthy When They Leave the Hospital

A transition of care occurs every time a patient's care changes hands - from a home setting to the emergency department, from one inpatient unit to another, from a primary care provider to a specialty care provider, from hospital to home.


The transition from an inpatient unit to home or a care facility is an important point in a patient's return to health. Older patients in particular can wind up right back in the hospital if the transition is handled poorly, so UW Health has implemented a program that informs patients of post-discharge care and communicates with patients about things that may signal health problems after they leave the hospital.
