The Lifestyle Change Program is a year-long commitment focused on helping you achieve your health goals.

  • Lower your risk of type 2 diabetes

  • Above all, learn about healthy behaviors to improve your quality of life

We welcome eligible patients from diverse health backgrounds, including cancer survivors, those who are pre- or post-organ transplant, as well as patients with heart disease or osteoarthritis.

How the program helps

By enrolling in the Lifestyle Change Program, participants will:

  • Connect with a certified Lifestyle Coach

  • Learn realistic strategies for improving everyday food choices, physical activity, sleep and well-being

  • Join a support network with others on a path to better health

  • Receive a free Dane County YMCA membership to use during our program

About the classes

Small group sessions

The Lifestyle Change Program supports you in making realistic changes to your lifestyle, including:

  • Nutrition

  • Physical activity

  • Sleep

  • Stress management

This small group meets weekly for the first 3 months, then biweekly for one month, and monthly for 8 months. Each class is one hour. Classes are offered virtually or in person. Classes are available on weekdays between 10 a.m. and 6:30 p.m.

Small groups are also available in Spanish.

Eligibility and cost

How to participate

Contact us for a full eligibility review. Participants must be patients of the UW Health system and have prediabetes or be at risk for prediabetes.

The program is often covered by insurance. Please contact your insurance provider and ask about coverage for the National Diabetes Prevention Program. The Lifestyle Change Program is a CDC-recognized Diabetes Prevention Program.

We invite partners, spouses, or support persons to enroll together, as this can provide support and accountability to foster success in the program.
