Dr. Jonathan Jaffery
Dr. Jaffery leads the UW Health ACO and provides strategic leadership for UW Health, where he is responsible for the overall development, coordination, and implementation of the population health strategy at UW Health. In collaboration with community partners, his role includes local network development, the UW Health population health care model development and deployment, and community health improvement efforts to promote the triple aim of better care, better health and lower costs.
A board-certified nephrologist, Dr. Jaffery holds a BA in Russian Literature from the University of Michigan, an MD from The Ohio State University College of Medicine, and completed an Internal Medicine Residency and Nephrology Fellowship at the University of Vermont. He also received an MS in Population Health Sciences from the University of Wisconsin School of Medicine and Public Health and a Masters of Medical Management from the University of Southern California Marshall School of Business.
Dr. Jaffery completed a Robert Wood Johnson Foundation Health Policy Fellowship where he worked for the United States Senate Committee on Finance, and is a former chief medical officer for the Wisconsin State Medicaid program.
