American Family Children's Hospital memorial program

The event will consist of a live program featuring a memorial video. The video will include each child’s name, years and photo along with your words of remembrance. In addition, there will be a print program that includes each child’s name and years. Please let us know how you would like your child to be honored using the form below.

We will accept RSVPs for families to view the ceremony past the April 29 deadline, but because of the length of time we need to produce the memorial video we may not be able to include your child in the ceremony. This deadline also allows us to mail the print program along with a special gift to you before the ceremony.
