Steps to Become a Volunteer
Thank you for your interest in volunteering. We appreciate your commitment to our patients, families and staff.
To become a volunteer, you must complete the following items:
1. Register for an Orientation Session
Register for an orientation by selecting the session you want to attend below and completing the registration form.
- Register for an American Family Children's Hospital orientation session
- Register for a UW Hospital and Clinics orientation session
After completing the online form, a confirmation note will appear with more details about the orientation. Please print and save this information for future reference.
2. Attend the Orientation Session
Please park in the parking ramp in front of UW Hospital and Clinics. At the orientation you will receive materials including an application, a health assessment form, a listing of available opportunities and a volunteer guidebook. Please do not mail, fax or return any forms to Volunteer Services prior to your placement interview.
3. Complete and Return the Health Assessment
After you complete the health assessment you received at your orientation, turn it in to Employee Health. Employee Health will review your records and notify Volunteer Services of your volunteer eligibility.
To make an appointment, call (608) 263-7535. Employee Health is located on the third floor at 800 University Bay Drive, Madison.
Once Volunteer Services is notified by Employee Health that you are approved to volunteer, Volunteer Services will contact you by e-mail or phone to schedule a placement interview.
4. Attend your Placement Interview
Bring your completed application and criminal background check and any remaining materials from orientation, including the guidebook. A finalized schedule and knowledge of your availability for volunteering is also required.
