Pre-Employment Requirements at UW Hospital and Clinics
All new employees of UW Hospital and Clinics are required to complete a pre-employment health assessment and new hire employee paperwork. Upon receiving an employment offer from UW Hospital and Clinics all new employees will be contacted by the Recruitment Center to schedule pre-employment and new employee orientation.
Please bring your completed Health Assessment Form (pdf)
and any documentation of immunizations or lab work to your appointment. You will also undergo a vision test, so please remember to bring glasses or contacts, if you wear them.

A urine drug screen will be conducted post-offer, pre-employment for specific safety-sensitive job titles.
New Hire Employee Paperwork
Please bring the following information to your Recruitment Center appointment:
- Two forms of identification are required to establish identity and eligibility to work in the United States. This information is required by the federal government to document employment eligibility. Copies of identification may not be accepted.
- Two emergency contacts, including name, address and phone numbers.
- Any licenses or certifications relevant to your employment.
All employees are also required to complete a Background Disclosure Form (pdf) and Disclosure and Authority to Release Information (pdf).
The Recruitment Center will send all new hire employee paperwork upon employment offer. This paperwork should be completed prior to your appointment with the Recruitment Center.
