Jacquelyn Byrd, NP, is a UW Health nurse practitioner who specializes in geriatric (over the age of 65) health care. 

When a patient needs to receive additional care before they can be discharged home, they will often be referred to a skilled nursing facility. While in the nursing facility, Jacquelyn will work with you and your primary care team to make sure your needs are met, address any new health concerns, manage medications, and help you transition to home when you are ready. She cares for patients with a wide range of conditions, including high blood pressure, heart failure, chronic kidney disease, respiratory conditions, urinary tract infections, wounds and skin infections. She also offers palliative care and hospice care. 
 
Jacquelyn comforts and cares for patients as they transition from the hospital to their nursing facility and back home again. She understands that transitions are difficult for many people. Jacquelyn collaborates with her colleagues as well as the patient’s family to ensure the best possible outcome for each individual.
