As you prepare to leave the hospital, your care team will give you your after-hospital care plan and medication checklist.
After hospital care plan
This printed care plan will help you understand what you will need to do after you leave the hospital, including treatments, follow-up appointments and/or labs. Please discuss any questions you have with your care team to make sure you fully understand this information.
Medications and filling new prescriptions before you are discharged
A pharmacist will review your updated medication list with you and explain the purpose, directions and side effects of each new medication.
Prescription and over-the-counter medications can be delivered to your room or picked up at the pharmacy of your choice.
- University Hospital: Monday–Friday, 8am–8pm; Saturday–Sunday, 8am–6pm
- UW Health at The American Center: Monday–Friday, 8:30am–5pm. During other times, your prescriptions and over-the-counter medications can be picked up at the 24-hour after-hours window.
UW Health pharmacies can continue to provide you with remarkable care and service that you experienced during your stay at UW Health. Please visit uwhealth.org/pharmacy for services.
After You are Discharged From the Hospital
Thank you for trusting us with your care. For your safety after you are discharged, remember to wash hands with soap and water, clean and disinfect frequently touched surfaces at your home and limit contact with others outside of your household.
