Advance Care Planning Classes
It's recommended you think about your health care wishes in advance, not when you are facing a medical crisis. UW Health is offering Advance Care Planning Classes to help you:
- Think about your healthcare values and goals
- Choose a health care agent (a person to make medical decisions for you in the event you are unable to make them yourself)
- Discuss your choices with your doctor and loved ones
- Make a written plan for the future, called an advance directive or Power of Attorney For Healthcare
These classes are free and we invite you to bring the person(s) you are thinking about appointing as your health care agent. Also, bring the addresses and phone numbers of up to three people you may want to appoint as health care agents.
The class will be an hour of content and 30 minutes set aside for individual questions or assistance with completion of the Power of Attorney for Healthcare document provided during class.
