ACE Plan of Care

UW Health's Acute Care for Elders (ACE) geriatric consultations can be requested by physicians before, during or shortly after a patient is admitted to the hospital.
The ACE team then collaborates with all health professionals involved in the patient's care to insure excellent care for geriatric patients. Working on all facets of care for the older adult, the team meets daily to discuss concerns about each patient, including common geriatric problems such as: 
  • Mental status changes (delirium, dementia, depression)
  • Mobility (falls and gait)
  • Nutrition
  • Pain
  • Incontinence/constipation
  • Skin integrity issues
  • Polypharmacy issues
  • Medication safety
  • Social concerns
  • Caregiver/living situations
