UW Health is offering this Advance Care Planning Class specifically designed for cancer and palliative care patients to:

  • Think about your healthcare values and goals
  • Choose a health care agent (a person to make medical decisions for you in the event you are unable to make them yourself)
  • Discuss your choices with your doctor and loved ones
  • Make a written plan for the future, called an advance directive or Power of Attorney for Healthcare

This class is free (parking will also be included) and we invite you to bring the person(s) you are thinking about appointing as your health care agent. Also, bring the addresses and phone numbers of up to three people you may want to appoint as health care agents.

 

The class will be an hour of content and 30 minutes set aside for individual questions or assistance with completion of the Power of Attorney for Healthcare document provided during class.

 

Two sessions for cancer patients are available on Tuesday, December 18:

  • Daytime: 9:30-11am
  • Afternoon: 1:30-3pm
