If you are planning to withdraw from a class or activity and would like a refund issued to the credit card with which you paid, please use this form. You are welcome to process withdrawals through the "My Account" area of your account; however, please be aware that those refunds will be issued as a credit added to your UW Health ACTIVE Network account.
