Per UW Health Policy 11.19, UW Health faculty and staff can place their names on a "Do Not Contact" list that serves as notice to all vendor representatives that they do not wish to be contacted by phone, fax, or e-mail. Faculty and staff members who do not wish to be contacted should complete the form below. Your name and department information will be added to the list within a week (of course, your e-mail address will not be provided in this list!).
Vendor representatives are expected to monitor this list carefully before contacting UW Health faculty or staff members without being contacted by them first. It is the representative's responsibility to NOT contact individuals whose names appear on this list. Failure to follow this policy will result in disciplinary actions as outlined in policy 11.19.
Placing your name into the Do Not Contact List is a permanent action. You will remain on the list indefinitely. If, at some time in the future, you wish to have your name removed from the Do Not Contact list, please contact the Vendor Liaison Office.
