Referring to Wisconsin Sleep

Sleep disorders present in a variety of ways. If you need additional resources to help your patients with their sleep problems, Wisconsin Sleep can provide clinical consultation as well as diagnostic testing and interpretation in a comfortable, patient-centered environment.

 

This requisition form (pdf) can be completed, signed and faxed to Wisconsin Sleep to initiate your referral. In addition, we require the following:

  • A recent clinic note describing the clinical issue pertaining to the patient’s sleep problem
  • A recent (within the past six months) physical exam
  • After gathering and reviewing the necessary information, clinical staff will evaluate for appropriate referral, acceptance of testing criteria, type of testing/monitoring needed and any exclusion criteria

The Wisconsin Sleep Clinic offers consultations to:

  • Evaluate sleep complaints or concerns
  • Recommend further testing or treatment
  • Discuss laboratory results
  • Review medication or home equipment plans
  • Follow up on previous visits and treatment plans
  • Offer patient education on sleep disorders 

The Wisconsin Sleep, Inc. laboratory offers diagnostic testing, including:

  • Polysomnography, including high-density EEG testing
  • Multiple sleep latency testing
  • Maintenance of wakefulness testing
  • Actigraphy
  • Portable respiratory monitoring
  • Auto-CPAP studies
  • CPAP, Bi-level PAP and servo-ventilator titration
