UW Health's Bariatric Surgery Program has produced the following video to help you learn more about the program and bariatric surgery.
Instructions
Watch all sections of the video below. You may review the sections in any sequence. However, the order in which the sections appear is recommended. You can hover over the thumbnail images beneath the main video screen to see that segment's topic, and select a thumbnail to begin the video.
After you have watched all of the video segments, select the link that opens our registration form and quiz if you are interested in beginning the bariatric surgery evaluation process. Note: The link to the registration and quiz page displays at the end of each video segment. You only need to complete the registration form and quiz once.

- Review the bariatric surgery Web site for further information.
- Call your insurance company to check your benefits. Do they cover bariatric surgery? What are the criteria?
- Ask your primary care provider to write a letter to our program supporting your decision to have bariatric surgery and outline your medical, surgical and weight loss history.
