1. You will be treated with respect and courtesy and will be free from all forms of abuse and harassment.
2. You have the right to privacy so that you are not seen or overheard during your treatment by people not involved in your care.
3. Your health care information will be treated as private and confidential. Details of your condition and treatment may not be shared except with those who are allowed to receive the information. To aid in your care when you are seeing health care professionals in more than one organization, health information that UW Health stores electronically is also available to health care providers associated with UW Health such as Group Health Cooperative and Meriter Hospital.
4. You can request restrictions on certain uses and disclosures of your health information according to the Federal Health Insurance Portability and Accountability Act (HIPAA). You can also request a correction of your medical record.

5. You have the right to request a limit on the number of medical students and residents involved in your care. One of UW Health's missions is to train the next generation of physicians. Our resident physicians and medical students appreciate the opportunity to participate in your care and advance their knowledge of medicine. Faculty physicians are responsible for your care as well as for the supervision of resident physicians and students. Patients who wish to limit the involvement of resident physicians or medical students with their care should speak with their doctor. To the extent possible, the request will be honored if doing so would not negatively affect the patient's care, treatment or services.
