Brain Tumor Medical Record Review
The University of Wisconsin Paul P. Carbone Comprehensive Cancer Center offers brain tumor patients the option of sending their medical records for review instead of coming for a clinic visit.
This is not a fee-for-service or a medical consultation. However, it does provide patients with a preliminary recommendation based on limited clinical data.
To Have Your Records Reviewed
Follow the instructions below to have your medical records reviewed by our multidisciplinary brain tumor specialists who will review your case.
- Contact Cancer Connect at (800) 622-8922 or (608) 262-5223 to request a medical records review, provide contact information and obtain mailing instructions.
- Gather the following medical records to send to our facility:
- MRI films and reports
- CT films and reports
- Documentation of any previous treatment such as surgery, chemotherapy or radiation
- Pathology report only (pathology slides are not needed)
- Other imaging scans and reports (e.g. spectroscopy or PET, if applicable)
- Recent blood test results
- You will be contacted in writing with our recommendations.
