UW Health Transplant Referrals

As part of the UW Hospital and Clinics academic medical center the UW Health Transplant Program has gained international recognition for leadership in organ preservation, transplant surgery and care of transplant patients.


The UW Health Transplant Program strives to provide superior communication, education and support to referring providers. The goal of the program is to work in a collaborative effort to provide extraordinary transplant patient care.


Transplant Program Referrals


To refer a patient for transplantation, you may contact the UW Health Access Center at 1-800-472-0111. If you prefer, you may contact the transplant program directly by telephone or fax a transplant patient referral form (pdf):

Kidney Allocation System


The kidney allocation system, managed by UNOS, will be changed on December 4, 2014. The kidney allocation system is the process used to determine which patients get offered donated kidneys. This document outlines the new system - FAQ: Upcoming Changes to the Kidney Allocation System (pdf)


Transplant Program Education and Outreach


The UW Health Transplant Program provides referring provider education programs to provider groups, dialysis centers and professional organizations around the Midwest. The transplant team can provide an educational or outreach program topic of interest to you.
