UW Health Sports Medicine Classic Registration/Donations

You can contribute to the Sports Medicine Classic in any of three ways listed below.

Option 1: Register and Pay Online

Go to https://donate.uwhealth.org/page/contribute/SMClassic and complete the registration form. After submitting the form you will be formally registered, and a Classic representative will contact you shortly thereafter.

Option 2: Donate Online

Go to https://donate.uwhealth.org/page/contribute/sportsmedicine and complete the donation form, making sure to indicate the amount of your donation.

Option 3: Register/Donate and Pay by Mail

Fill out our online information form. After submitting the form, a Classic representative will contact you shortly thereafter to arrange for details.
