Health Insurance
 
Residents can choose from a variety of comprehensive health plans, including fee-for-service plans and HMOs. The application form and It's Your Choice booklet describing the different plans will be included in the new hire packet.
 
All HMO plans include:
  • 100% coverage 
  • Basic dental coverage; cleanings, X-rays, and fillings every 6 months and orthodontia coverage to age 19
  • No deductibles 
  • Co-pays only for prescription drugs; $5 generic, $15 name brand and $35 non-formulary 
    • Annual out-of-pocket maximum is $350 individual and $700 family
  • Single or family coverage 
  • Minimal monthly premiums

Delta Dental - Supplemental

 

Delta Dental provides supplemental dental coverage with maximum coverage of $1,000 per year. There is a minimal monthly premium and a $25 or $50 annual deductible for services.

Coverage includes:

  • 100% of diagnostic and preventive services 
  • 80% of basic restorative services 
  • 50% of major restorative services, including crowns, bridges 
  • 50% of orthodontics to age 19 (Maximum benefit of $1,000)

There are more than 450 dentists in the statewide-preferred network.

 

Spectera Vision Insurance

 

Spectera Vision Care provides supplemental coverage for eye exam, lenses, frames and contacts. Frames, lenses, and contacts either free of charge or significantly reduced every 12 months. Coverage is also available for spouse and children. There is a minimal monthly premium and a $25 co-pay.

Epic Major Medical and Dental - Supplemental

 

This program provides supplemental health and major dental coverage to employees and their dependents. EPIC Major Medical and Dental coverage is intended to supplement, not replace, your primary health coverage.

Employee Reimbursement Account (ERA) for Medical Expenses

 

The employee reimbursement account allows employees to pay for medical and child care expenses with pre-tax income. The maximum annual contribution for medical expenses is $7,500; the minimum annual contribution is $100.

 

This account covers medically necessary expenses not covered by health insurance such as:

  • Prescription co-pays
  • Major dental expenses
  • Glasses or contacts
  • Refractive surgery
  • And many over-the-counter products

The maximum annual contribution for dependent care is $5,000; the minimum annual contribution is $100. The account covers the expenses for daycare, as well as before and after school care of children.

 

The application for the ERA will be included in the new hired packet along with Its Your Choice booklet.
