Sports Rehabilitation Self Referral (Direct Access to Physical Therapy)
UW Health Sports Rehabilitation is pleased to offer direct access to licensed physical therapists without the need of a physician referral.
Physical therapists are an important member of your health care team. They have extensive training and skills in evaluating and treating your injury or condition. They provide screening to ensure that physical therapy is the most appropriate method of care for you.
Physical therapy services can be used for diagnosis, treatment and prevention of impairments, functional limitations and disabilities related to movement, function and health. Treatment starts with a comprehensive physical evaluation and detailed assessment. The outpatient rehabilitation staff focuses on treating your current symptoms as well as the factors that may have precipitated your injury or condition. They then design a rehabilitation program to fit your specific needs.
Physical therapy services are beneficial for a variety of surgical and non-surgical musculoskeletal conditions and injuries, including:
- Acute sports injuries
- Acute and chronic neck or back pain
- Injuries of the arms or legs
- Joint stiffness and muscle weakness
- Muscle strains and joint sprains

Your physical therapy reports will be entered into your medical records. You will be referred to a physician if it is determined that your condition requires further evaluation.
