Whether you would like to seek the opinion of a UW Health physician, transfer a patient to the University of Wisconsin Hospital and Clinics or check on your patient's status, we are here to serve you.
We recognize the importance of the relationship that you have with your patient and seek to honor that relationship by providing the best possible care and keeping you apprised of our progress.
UW Hospital and Clinics and the American Family Children's Hospital offer services at 80 different specialty clinics. In order to assist you with referring a patient, we have streamlined the referral process by creating one form and one fax number for you to use to request an appointment:
