How to Request Outpatient Clinic Appointments
 
UW Hospital and Clinics and the American Family Children's Hospital offer services at 80 different specialty clinics. In order to assist you with referring a patient, we have streamlined the referral process by creating one form and one fax number for you to use to request an appointment:
The Access Center: Help with Admissions and Referrals
 
UW Hospital and Clinics has created the Access Center to handle all issues surrounding patient admissions and referrals. Referring physicians can use the Access Center to:
  • Obtain rapid consultations with specialty staff
  • Refer patients on an outpatient or inpatient basis
  • Get assistance in scheduling clinic appointments
