Billing and Patient Records Guide

UW Health clinic personnel will submit all claims to your insurance. You will be billed for any balance due after insurance has paid. We accept all indemnity insurance plans, as well as the following:

  • Medicare: We accept Medicare assignment

     

  • Medical Assistance: We accept Medical Assistance patients, but do not participate in all Medical Assistance managed care plans. If you are covered under a managed care or out-of-state Medical Assistance plan for which UW Health is not a participating provider, you will be directed to a participating provider, asked to obtain an appropriate referral, or requested to sign a financial responsibility form.

     

  • Managed Care Plans: We accept Unity and PPIC, as well as a number of other managed care plans. If you are covered under a managed care plan for which UW Health is not a participating provider, you will be directed to a participating provider, asked to obtain an appropriate referral or requested to sign a financial responsibility form.

     

  • Worker's Compensation: If you are being treated for a work-related injury, please inform us at the time of service. We will complete the necessary forms and forward them directly to your employer or insurance company.

You will receive a monthly statement from UW Health Physicians once your insurance has paid and the remaining balance has been determined to be your responsibility.

 

For added convenience, you can pay all of your UW Health bills on this Web site with a single transaction using our online Bill Pay tool.
