If you would like to request copies of your radiology images but are unsure whether your clinic is managed by UW Hospital and Clinics or the UW Medical Foundation, please refer to the Obtaining Medical Records: UW Health Clinics Listing to find out where to direct your inquiry.

 

To obtain copies of radiology images that were taken at a UW Medical Foundation clinic, you must contact the clinic directly and download and complete the UW Medical Foundation Medical Records Authorization (pdf).

 

To contact the UW Medical Foundation clinic where your images were taken, please consult the UW Health clinics listing above to obtain a phone number to call.

 

There is no charge to mail radiology images to other hospitals, clinics or physicians for further medical care. The charge is $22 per copy for images that are requested for other reasons, and payment must be made in advance of copying.
