If you would like to request copies of your radiology images but are unsure whether your clinic is managed by UW Hospital and Clinics or the UW Medical Foundation, please refer to the Obtaining Medical Records: UW Health Clinics Listing to find out where to direct your inquiry.

 

You can request copies of your medical images for radiology procedures performed at UW Hospital and Clinics by downloading and completing the UW Hospital and Clinics Medical Records Authorization (pdf) and mailing or faxing the form to the Image Management section of the radiology department at the following address:

There is no charge to mail radiology images to other hospitals, clinics or physicians for further medical care. The following charges apply to copies requested for other reasons:

 

 

Burned to a CD: $24.30 per CD

 

 

35 inches by 43 inches: $28.50 for the first copy and $14.90 for each additional copy

 

24 inches by 30 inches: $21.50 for the first copy and $8.10 for each additional copy

 

All personal charges for radiology images will be processed directly to your patient account.
