Obtaining Medical Records from UW Medical Foundation
If you would like to request copies of your medical records but are unsure whether the clinic is managed by UW Hospital and Clinics or the UW Medical Foundation, please refer to the Obtaining Medical Records: UW Health Clinics Listing to find out where to direct your inquiry.
You can request copies of your medical records for clinics managed by the University of Wisconsin Medical Foundation by downloading and completing the UW Medical Foundation Medical Records Authorization (pdf) and mailing or faxing it to the Health Information Department at the following address:

There is no charge for copies of your medical records that are mailed to another physician or health care facility. If you are requesting a copy of your most recent visit for yourself (one to three pages), there is no charge. If you are requesting your medical records for your personal use, you will be sent a fee approval notice.
When we receive your acknowledgement to proceed, records will be processed and you will be billed at the following rates for paper copies:
- $1.01 per page for the first 25 pages
- 76 cents per page for pages 26-50
- 51 cents per page for pages 51-100
- 31 cents per page for pages 101 and above
