Donor Family After Care Program
The donor family follow-up program at UW Organ and Tissue Donation serves donor families and families of Donors in Spirit.
The formal program includes five points of contact between the donor family and UW Organ and Tissue Donation during the 15 months following donation, however our services are available for donor families as needed throughout their grief process.
Within the First Month
- A phone call from UW Organ and Tissue Donation to check on your family
- Information about your loved one’s donation
- Support materials, including a letter from the UW Organ and Tissue Donation Executive Director and a certificate from the Governor of Wisconsin
3 and 4 Month
- A phone call from UW Organ and Tissue Donation to check on your family
- Information about our online donor tribute service
- A bereavement resources packet
9-10 Months
- A grief support book
15 Months
- A phone call from UW Organ and Tissue Donation to check on your family
- A survey will be sent to you. Your feedback is very important and allows us to better assist other donor families
