Patients and Visitors: A guide for hospital stays and clinic visits

Welcome to UW Health. We are pleased to have the opportunity to serve you. As a new patient, please review the following information to help you answer your questions about accessing care at UW Health facilities.
 
Selecting a Primary Care Provider or Finding a Specialist
 
If you are searching for a primary care provider or specialist, you are welcome to use our Find a Doctor directory.
 
The Advanced Search allows you to select a subspecialty and/or enter additional search criteria. You may also browse by clinic location or enter a specific procedure or condition.
 
If you would like personal assistance selecting a physician, please call (608) 821-4819. Our Patient Resources staff would be happy to assist you in finding a provider based on your individual needs.
 
Registration and Check-In
 
After your appointment has been scheduled, you will be transferred to a registrar who will gather or confirm your demographic and insurance information. We do this to:
  • Save you time when you arrive for your visit
  • Ensure that your personal information is gathered as privately as possible

Please plan to arrive 15 minutes prior to your scheduled appointment to check in. If you are pre-registered, we will only confirm your name and date of birth. If you have not been pre-registered, you will be asked to confirm your demographic and insurance information.

 

Copayments, if required by insurance, will be collected when you arrive and may be made by cash, check, VISA or MasterCard (credit or debit).

 

Preparing for Your Clinic Appointment

 

We want you to get the most out of your clinic appointment. Follow these tips to better prepare for your appointment:

  • Write down what you would like to discuss with your provider, including any questions
  • Know the names of all of your current and past medications. You may use the Preparing For Your Appointment (PDF) form to record your medications and what you would like to discuss with your provider.
  • If you are being treated at UW Hospital and Clinics, understand our Policy on Patient Belongings and Valuables
  • Know what symptoms you are experiencing, when they started and how to clearly describe them
  • Bring information regarding your personal health history and the health history of your family
  • Write down all dates and names of procedures, tests and medications pertaining to your health history
  • Be prepared to talk about all other remedies you use to control your health such as vitamins, nutrition, exercise, meditation and counseling
  • Before you leave, make sure you fully understand your physician's recommendations and answers to your questions. Also, speak with your physician regarding the timing of any necessary tests or follow-up visits.

Getting to Your Appointment

Billing Information

 

We will submit all claims to your insurance. You will be billed for any balance due after insurance has paid. We accept all indemnity insurance plans, as well as the following:

  • Medicare: We accept Medicare assignment

     

  • Medical Assistance: We accept Medical Assistance patients, but do not participate in all Medical Assistance managed care plans. If you are covered under a managed care or out-of-state Medical Assistance plan for which UW Health is not a participating provider, you will be directed to a participating provider, asked to obtain an appropriate referral, or requested to sign a financial responsibility form.

     

  • Managed Care Plans: We accept Unity and PPIC, as well as a number of other managed care plans. If you are covered under a managed care plan for which UW Health is not a participating provider, you will be directed to a participating provider, asked to obtain an appropriate referral or requested to sign a financial responsibility form.

     

  • Worker's Compensation: If you are being treated for a work-related injury, please inform us at the time of service. We will complete the necessary forms and forward them directly to your employer or insurance company.

You will receive a monthly statement from UW Health Physicians once your insurance has paid and the remaining balance has been determined to be your responsibility.

 

For added convenience, you can pay all of your UW Health bills on this Web site with a single transaction using our online Bill Pay tool.
