10. How do I remember all of my discharge information?
 
To avoid running out of medications, plan ahead. You need to know where you will get your refills and how you will pay for them.
 
When you are stressed, it is helpful to have a ready list of things to do and people to call for help. Take time now to list activities that soothe you and help you to relax. The things you list should be pleasant. Write down your family members or your support persons that you can call in a time of need. Think about who is most helpful to you.
 
Use My Discharge Information (pdf) and use the lines available to record your answers. Remember that these are for your own use and will not be stored in our records. Then, keep the document as a reference for when you need to remember any of your discharge information.
