6. Do you have a follow-up appointment?
 
Getting to your follow-up visit is essential to your health and well being.
 
Talk with your doctor about your follow-up clinic visit. Your nurses will give you a discharge sheet with the date and location of your next clinic visit. This visit is a time for you to talk with your doctor about how you are doing at home, renew medications and get your questions answered.
 
It is important for you to know the address and phone number of the clinic where you have an appointment.
