You can request copies of your medical records for clinics managed by the University of Wisconsin Medical Foundation by downloading and completing the UW Medical Foundation Medical Records Authorization (pdf) and mailing or faxing it to the Health Information Department at the following address:

There is no charge for copies of your medical records that are mailed to another physician or health care facility. If you are requesting a copy of your most recent visit for yourself (one to three pages), there is no charge. If you are requesting your medical records for your personal use, you will be sent a fee approval notice.

 

When we receive your acknowledgement to proceed, records will be processed and you will be billed at a rate of $0.31 for each page plus postage and appropriate sales tax.
