You can obtain medical records and copies of radiology images by printing and completing one of two authorization forms and mailing or faxing it to the proper organization.

 

The form and contact information differ depending on whether you received care from a UW Hospital and Clinics location or a UW Medical Foundation (UW Health Physicians) location.

You may also obtain pathology reports and slides by contacting the Pathology department. Please contact us if you have questions about obtaining your medical records.
