Teen Opportunities

 

If you'd like to volunteer or are interested in a community service project, there are several opportunities especially for teens.

Thank you for your interest in volunteering. We appreciate your commitment to our patients, families and staff.

 

To become a volunteer, you must complete the following checklist of orientation activities:

 

Becoming a Volunteer 

  • Register with the volunteer office at volunteer.services@uwhealth.org for a general orientation of your choice. Please include your name and a contact phone number.
  • At the orientation, you may have the opportunity to set up a placement interview.
  • Turn in completed Health Assessment Form to Employee Health and have tuberculosis (TB) skin test requirements done.
  • Attend your placement interview with your completed application forms and your calendar. You may have your photo ID taken at this meeting.
  • Any new teen or college student volunteers will have to provide proof of the H1N1 vaccination.  

Please note: When you register, if you do not receive a confirmation reply within one week of sending the e-mail, please call the Volunteer Office at (608) 263-6046.

 

Paperwork

 

In order to become a volunteer, you'll need to complete the following forms:

General Orientation Session

 

To register, please call the Volunteer Services office at (608) 263-6046 or e-mail volunteer.services@uwhealth.org. Please provide us with your daytime phone number and the date of the orientation you'd like to attend. When you come to the orientation, if you park in the patient ramp, bring in your ticket. We will validate your parking ticket at no cost to you at the Information Desk when you identify yourself as a volunteer coming for orientation.

UW Hospital and Clinics Volunteers:

  • Nov. 16: Orientation for UW Hospital and Clinics, 5-6:30pm
  • Dec. 15: Orientation for UW Hospital and Clinics, 9-10:30am

American Family Children's Hospital Volunteers:

  • Jan. 11: 5:30-7:30pm
  • Feb. 8: 5:30-7:30pm
  • March 8: 5:30-7:30pm
  • April 12: 5:30-7:30pm
  • May 10: 6-8pm 

Employee Health Requirement

 

You will need to go to Employee Health for at least one tuberculosis skin test and to review your health history form. Make sure you have all your immunization dates completed on the form before going for your tuberculosis skin test. This will expedite you being cleared to volunteer. If you have questions as to what is required, please ask one of the nurses in Employee Health Services. All requirements must be completed before you may begin volunteering in your service area.

 

If you have not had a tuberculosis skin test within the last six months, you will be given one by Employee Health. If you have never had a tuberculosis skin test, or have not had one within the past five years, you will be required to be tested twice. The tests are free to UW Hospital volunteers. If you have had TB testing before, bring in the documentation of the tests with your health form.

Placement Interview

 

During the general orientation, you will be given an opportunity to sign up for a placement interview with Volunteer Services staff.

 

Please attend your placement interview in Room E5/720Q (the Volunteer Services office) for the adult hospital. Please bring your completed volunteer application to the interview. During this interview, we will place you in a volunteer position according to your schedule and interests. At the interview, your photo will be taken for your photo identification badge.
