Overview
UW Health Belvidere Hospital is an acute and outpatient care hospital offering a 24/7 Emergency Department, as well as the convenience of imaging, physical therapy and rehab, lab and more, with several walk-in services available.

Walk-in hours: Monday–Friday, 7 a.m.–6 p.m.; Saturday, 7 a.m.–noon
Requirements: Patient order from provider (you can bring it in with you, fax it or have your provider submit it)
Front desk will ask you for basic demographic information and if you are fasting
X-ray
Walk-in hours: Monday–Friday, 7 a.m.–6 p.m.; Saturday, 7 a.m.–noon
Requirements: Patient order from provider (you can bring it in with you, fax it or have your provider submit it)

Mammography
Walk-in hours: Monday–Friday, 7 a.m.–2 p.m.
Requirements: Must be at least one year since last mammogram
Front desk will ask you for insurance information or will provide you with the health department’s contact information. They will also ask who your results should go to (primary care provider, OB-GYN, etc.) and if you are having any concerns related to your breast health.
How to find us
Emergency Department, outpatient testing and imaging: Use the entrances on the west side of the building (turn in from 9th St or enter the parking lot off of South State St and go around to the back of the hospital).
Physical therapy, rehabilitation and sleep studies: Use the clinic entrance on the south side of the building (South State St entrance).
