| Collection Instructions: |
|
Slides or paraffin-embedded blocks should be placed in a suitable transport container. Outside pathology report must be included with slides/paraffin-embedded blocks.
Requests must include patient name, date of birth, medical record number (for internal customers), referring physician, and referring institution. A letter requesting the review should accompany any request from a physician outside UW Health.
