Test Code(s): AFB / HCAFB

CPT Code(s): 87015, 87206, 87116

Test Description: Test Component:

Acid fast stain and culture with identification for mycobacteria. Species identification may include HPLC, DNA probes, biochemical testing, growth characteristics, and gene sequencing. Mycobacterial susceptibility testing available on species isolated.

Methodology: Microscopy and Culture

Testing Site: Testing will be sent to an approved reference laboratory.

Days Performed: Mon-Fri.

Turnaround Time: Routine: Smear/Stain: 1 - 3 days. Final report: 1 - 8 weeks.

Specimen Requirements

Specimen: Cerebrospinal fluid (CSF), body fluids, bone marrow, stool, tissue , skin lesion material

Patient Preparation: Collect specimens prior to chemotherapy.

Collection Instructions: Sterile preparation of collection site if applicable. Follow packaging instructions included with State Lab Acid Fast Bacillus (AFB) collection kit (CS Item# 2200232). Swabs not recommended as negative results obtained from specimens submitted on swabs are not reliable. All requests for AFB Smear and Culture on CSF will require approval by the Director of Microbiology before being fowarded to WSLH for testing.

Specimen Volume
CSF: >= 1.0 mL
Body Fluid: 5-10 mL
Bone Marrow: as much as possible in SPS blood collection
              tube
Stool: >= 1.0 gm
Tissue: 1.0 gm; NO saline, NO swabs or gauze for
          transport
Skin Lesions Material: as much as possible; NO saline, NO
          swabs or gauze for transport

Collection Container: Preferred: State Lab Acid Fast Bacillus (AFB) collection kit
Also Acceptable: Sterile screw cap container

Collection Volume: Preferred: See collection instructions.

Specimen Transport: Transport specimen to UWHC Core Laboratory (B4/220).
Transport to laboratory as soon as possible and refrigerate if delay in submitting. Transport with coolant pack if coming from outreach location.

Interpretation

Additional Information:

Culture results include:  Negative; Species identification when mycobacterial growth is detected; Culture overgown with non acid-fast organisms.

Mycobacterial susceptibility testing recommended for M. tuberculosis isolates.

All requests for AFB Smear and Culture on CSF will require approval by the Director of Microbiology before being fowarded to WSLH for testing.
