Applying to the Program

Critical Care Specialty Residency
 
In order to be considered for the Critical Care Specialty Residency at the University of Wisconsin Hospital and Clinics, you must:
  1. Have completed a pharmacy practice residency by July 1. 
     
  2. Be enrolled in the American Society of Health System Pharmacists Residency Matching Program. Applications may be downloaded from the ASHP Web site.  The deadline for match registration is usually in mid-January.
     
  3. Complete an online application by going to the UW Hospital and Clinics online job application. Apply for job ID 99005. The application requires completion of an online behavioral assessment. Your online application will be rejected if you do not complete the assessment. Take your time to read each question carefully and answer as accurately as possible.
     
  4. Submit an application packet by January 15, 2010 containing the following:
     
    • Letter of intent. Include the following in your letter:
      • Why do you want to complete a critical care residency? 
      • Why do you want to complete a critical care residency at UW Hospital and Clinics? 
       
    • UW Hospital and Clinics Critical Care Residency Application (see Application Materials Packet
       
    • Curriculum vitae 

      Mail application packets to:

  5. The three persons you select as references should mail their recommendation forms (see Application Materials Packet) directly to Jenny Lendborg at the address listed above.
     
  6. Request a copy of your official pharmacy school transcripts; have them sent directly to Jenny Lendborg at the address listed above.
     
  7. Once your complete application packet is received, you will be contacted by the department secretary to set up an interview. You may also contact the critical care program director, Jeff Fish, at (608) 263-1290 or jfish@uwhealth.org with any questions about the interview or the program.
