When filling out the Graduate School Application for Admission, use the information provided below:
  1. Submit Graduate School Application for Admission (linked above) 
    • 1A - First semester/year in which you wish to enroll: should read Fall (yr): year in which you enroll; full time
    • 1B - Intended Major Field of Study: should read PHARMACY, select Pharmacy MS
  2. GRE's (general aptitude only) must be taken by March following the application deadline
  3. Reasons for graduate study (state reasons for graduate study, why you want to participate in the residency program your special interests in research area, and indicate your background preparation, 1-2 pages)
  4. List references in application, but it is not necessary to submit their e-mail addresses

  1. Copy of Graduate School Application for Admission
  2. Two sets of offical transcripts of all college coursework
  3. Copy of reasons for graduate study
  4. Curriculum Vitae
  5. Three Letters of Recommendation (pdf)
    (One letter must be an academic recommendation and one must be from an employer or boss.)
Application Deadline
 
To be considered for fall admission, all application material must be received by January 15.
