Orthopedic Clinic Preoperative Check List - Total Hip Replacement
Please be sure to set up your physical therapy appointments at your local place. These visits should begin 10 – 14 days after your surgery date.

Orthopedic Clinic Preoperative Check List
______ Stop at Admissions. It is behind the information desk at the Main
Hospital Entrance. If there is time, you may go there before your
Orthopedic Clinic visit.
______ Meet with a Physician Assistant in the Orthopedic Clinic H4/2.
______ Attend a class about total joint replacement surgery - you will be
scheduled for one of these:
______ A Learning Center class in the Orthopedic Clinic H4/2 or
______ A class on the Orthopedic In-patient Unit B6/4. Take the B
elevators to 4th floor.
______ Meet with a Social Worker in the Orthopedic Clinic H4/2.
______ Make a 2 week post-op appointment at the Orthopedic Clinic H4/2 front
desk.
Your Physician Assistant or Clinic Nurse will tell you if you need to have any tests, x-rays, or labs drawn or meet with the Anesthesia Nurses (locations below).
______ X-ray is in J5/2. This is just past the Orthopedic Clinic Entrance.
______ Clinics Lab is in G5/2. This is within the Orthopedic Clinic H4/2 area. It
is also in the same area as Outpatient Surgery near the Clinics
Information desk.
______ EKG (electrocardiogram) is in the Heart & Vascular Clinic. Take the
Atrium Way elevators to the 4th floor to get to this clinic.
______ Anesthesia Nurses is in G5/2. This is in the Outpatient Surgery
Center near the Clinics Information desk.
Please come for your visits at your scheduled time. The visits may take 4-6 hours. Feel free to eat before you come. Also, please take your normal medicines.
Please read your Patient’s Guide to Total Hip Replacement booklet to help you prepare for your clinic visits. Please write down any questions you have and bring them with your booklet to your clinic visits.

Things to bring with you:
- Your insurance information
- Your HMO referral, if needed
- A list of your medicines, including name, dose, frequency and any herbs and natural remedies
- Information about recent physical exams, blood work, x-rays, EKG, or other tests
- Pre-operative Folder Information
