Orthopedic Clinic Preoperative Check List

______ Patient Education – Learning Center H4/2

______ EKG (electrocardiogram)
______ Chest X-ray
______ Admissions (Behind information desk at Main Hospital Entrance). If time
allows, you may go there before your Orthopedic Clinic visit.
______ Anesthesia Nurse, G5/2 (in Outpatient Surgery near the clinics
information desk)
______ Clinics Lab, G5/2, (same area as Outpatient Surgery)
Please come to these visits at the scheduled time. It may take 4-6 hours. Feel free to eat before you arrive. Also, take your normal medicines.
Enclosed is the Patient’s Guide to Total Knee Replacement booklet to help you prepare for this clinic visit. Please read it, write down any questions, and bring the booklet and questions to your clinic visit.

Things to Bring with you
- Your insurance information
- Your HMO referral, if needed
- Your medicines, including herbs and natural remedies
- Information about recent physical exams, blood work, x-rays, EKG or other test
