Pediatric Cardiac Pre-Surgery Check Sheet

 

This information will help direct you to the correct departments as you prepare for your child’s surgery.  Please make sure that by the end of the day you have completed all the needed visits and all of your questions have been answered.


First Stop


____ Learning Center- Check in at the Guest Depot on the main floor and ask for the Learning Center Nurse to be paged. Wait at the tree to meet the nurse to receive your pre-surgery teaching. The Learning Center Nurse will then direct you to the second floor to check in for clinic.

 

American Family Children’s Hospital (AFCH) 2nd Floor

 

____ Pediatric Cardiac Surgery Clinic- located within the AFCH Specialty Clinics. You will meet the cardiac surgery nurse practitioner for your child’s physical exam.
____ Anesthesia- During your clinic visit with the nurse practitioner, you will also meet an anesthesiologist.
____ Outpatient X-Ray- located within the AFCH Specialty Clinics. Your child will get a chest x-ray and other films he/she may need.  Hours: Monday – Friday  from 8:00 am to 5:00 pm
____ Outpatient Laboratory- located within the AFCH Specialty Clinics. Your child will get blood and urine samples.  Hours: Monday – Friday from 8:00 am to 5:00 pm
____ Child Life- You will be given the pager number for your Child Life Specialist. After lab and x-ray, ask reception to page her and she will meet you in the family waiting area. She will give you a tour of the hospital and discuss what to expect.

 

American Family Children’s Hospital 1st floor


____ Admissions- located in room 1342 across from the chapel. You will verify your insurance information.  Hours: Monday-Friday 8:30am to 4:00pm
