Supplemental Life Insurance - Term
Supplemental Life Insurance is available for yourself, your spouse or domestic partner and your dependent children.
Benefits Summary
- Life insurance can be purchased in increments of:
- $ 5,000 - $ 20,000 Employee
- $ 5,000 - $ 10,000 Spouse/Partner
- $ 2,500 - $ 5,000 Children
- Once enrolled there is an annual option to increase.
- Maximum level available anytime through evidence of insurability (underwriting):
- $ 200,000 Employee
- $ 100,000 Spouse/Partner
- $ 10,000 Children
- Premiums are based on the level of insurance selected and the age of the employee.
- Premiums will adjust according to age.
- Premiums are paid entirely by the employee and are taken as an after-tax deduction.

Accidental Death and Dismemberment Insurance (AD&D)
- Accident Insurance Protection: For loss of life coverage for you, your spouse, and children
- Permanent and Total Disability Protection: Payment in the event you are wholly and continuously disabled for the remainder of your life
- Dismemberment Protection: Payment for loss of sight and limb for you, your spouse, and children
AD&D coverage is intended to supplement, not replace, primary life insurance
