VSP Vision Care

 

If you do not want to make changes to your vision insurance, no action is necessary.

 

Open Enrollment: October 8–November 2, 2012

  • This is an opportunity to enroll, even if you currently do not have coverage.
  • Any enrollments and changes submitted during the open enrollment will be effective January 1, 2013.
  • You will not receive an insurance card. When receiving services you should inform the provider/merchant that you are covered by VSP insurance.

Benefits

 

VSP Vision insurance provides partial coverage for a vision exam plus eyeglass lenses every calendar year and eyeglass frames once every other year. The plan provides partial coverage for contact lenses every year instead of eyeglasses or eyeglass lenses. It also offers discounts on laser vision correction. You may see the provider of your choice but your out-of-pocket costs will be lower if you use a VSP provider. See the 2013 VSP Benefit Summary for more details.

Enrollment Materials

 

An application packet will be mailed to all eligible employees at their home address. Applications must be submitted directly to VSP by mail, phone or online. If you wish to cancel coverage, please contact VSP directly or submit an application to Human Resources by December 1, 2012 to be effective December 31, 2012. Please note that coverage cancellations received mid-year will be effective December 31.
