If you wish to cancel coverage, submit an application to Human Resources by December 1, 2012 to be effective December 31,2012. Please note that coverage cancellations received mid-year will be effective December 31.

Late enrollments are accepted throughout the year but there is a 12-month waiting period for Basic and Major Services and a 24-month waiting period for Orthodontic Services. Please note that any applications received for coverage without documentation of a Qualifying Event will be considered late enrollments.
